Application to Participate as a Student
This document is for persons seeking to participate in the activities of Machou Ryu Karate Do Seidokan Dojo Pty Ltd (the “Dojo”).
For most people, the moderate or vigorous physical activity encountered in training at the Dojo should not pose any problem or hazard.
However, if you are aware that such activity might be inappropriate for you due to a known medical condition, please inform the teacher of any precautions that you and teacher should take into consideration whilst engaging in the training activities of the Dojo. In such cases it is suggested that you consult a medical doctor and seek confirmation that you able to participate, in a full or conditional basis, in the activities of the Dojo. To assist the doctor, you should provide a copy of this application form as well as the Dojo’s Code of Conduct, specifically sections 2, 3, 4, 7, 8, 9 & 10.

Informed Consent and Conditions
In these conditions, “the Dojo” shall mean and include Machou Ryu Karate Do Seidokan Dojo Pty Ltd, its officers, servants, agents, teachers or authorised volunteers.
It is a condition of participation in the training conducted by the Dojo that you read and sign the informed consent and conditions below where indicated.

Checklist: 1. I am 18 years of age or over as at my initial training date;
2. I have informed the teacher of any precautions that you and teacher should take into consideration whilst engaging in the training activities of the Dojo;
3. I have read the Application to Participate as a Student and Code of Conduct documents;
4. By signing this form I have read and understood and agreed to all of the above.

- I hereby agree to abide by the Code of Conduct of the Dojo;
- I acknowledge that my participation in training conducted by the Dojo carries with it a real risk of personal injury;
- Except to the extent that the Trade Practices Act 1974 (Cth) or any similar state legislation imposes and condition, warranty, right or remedy which cannot be modified or excluded*, I acknowledge and agree the Dojo shall not be liable whether in tort or contract or otherwise for any loss or damage or injury whatsoever to my person or property whilst participating in training organised or conducted by the Dojo however caused or occurring including but without limiting the foregoing, the negligence, breach of contract, wilful act or default of the Dojo and whether or not such loss or damage or injury to me occurs within or away from any property occupied by the Dojo; and
